By signing this form, I acknowledge and agree to the following:
1. Voluntary Participation: I am voluntarily bringing my dog(s) to PDX-DOGPARK facilities and understand
that use of the dog park, daycare, training areas, and related services involves risks to me, my dog(s), and others.
2. Health & Behavior of My Dog(s): I confirm that my dog(s) are up-to-date on required vaccinations, are in
good health, and are free from contagious conditions. I confirm that my dog(s) are not aggressive, or if aggressive,
I have purchased the required separate facility space as outlined in the Terms and Conditions.
3. Assumption of Risk: I understand that dog interactions can result in injuries, illness, or property damage.
I voluntarily assume all risks associated with my participation and my dog(s)’ participation at PDX-DOGPARK.
4. Release of Liability: I hereby release, discharge, and agree to hold harmless PDX-DOGPARK, its owners, staff,
volunteers, and affiliates from any and all claims, damages, losses, or expenses resulting from my use or
my dog(s)’ use of the facilities. This waiver applies to injuries or damages to myself, my dog(s), or my property,
as well as injuries or damages my dog(s) may cause to another person, dog, or property.
5. Responsibility for Damages: I accept full responsibility for any harm or damage caused by my dog(s) to another
dog, person, or property while at PDX-DOGPARK.
6. Compliance with Rules: I agree to follow all PDX-DOGPARK rules, regulations, and staff instructions.
I understand that failure to follow rules may result in suspension or termination of access without refund
